Insurance and Health Plan Information

We work with most insurances & accept most plans within their networks with a few exceptions. Please check with your insurance prior to your appointment to confirm your benefits, immunization coverage, & any specifics regarding your care.

Verification of benefits is not a guarantee of payment, and you are ultimately responsible for payment of all services that are not covered by your insurance plan once the claim has been processed.

 

Plans that LFM does not accept:

• Any exchange/Market Place Plans

• AHCCCS

    

Please bring your insurance card and a photo ID to your visit. It is very important for you to bring your current insurance card to each visit and inform us if there have been any changes in your coverage or in your address and phone number.
